Get Vaccinated Against COVID-19

Submit a request to be assigned a vaccination appointment. A phone call or email confirmation for the vaccination appointment will be delivered within 24 hours following submitting your contact information. Submissions sent on Friday or over the weekend will be addressed on Monday. Appointments are scheduled in sequential order. Though you will be able to choose which vaccine, you will not be able to choose the time of day for your appointment. This is due to the requirements for managing the doses.
