BYU–Hawaii is committed to safeguarding the campus and community. We must work together to protect each other from the spread of COVID-19. One way to accomplish this is to cooperate with our active monitoring efforts on campus.

Students and employees are strongly encouraged to complete a daily wellness check by asking themselves these questions:

  • Did I wear a face-covering today and did it fit snugly against the sides of my face?
  • Did I practice physical distancing (staying six feet apart from others)?
  • If I didn't feel well, did I stay home?

If you answered yes to all those questions, thank you for doing your part!

In the past 24 hours, have you experienced any of the symptoms of COVID-19 listed below?

  • Change or loss of taste or smell.
  • New or worsening cough, shortness of breath.
  • Temperature greater than 100F.
  • Sore throat, sneezing, runny nose.
  • Muscle aches, fatigue.
  • Nausea, vomiting, diarrhea.
  • Headache or "brain fog."
  • Spent more than 15 minutes within 20 feet of someone who has tested positive for COVID-19 in the past 14 days.
