BYU-HAWAII ALUMNI NOMINATION FORM The BYU-Hawaii/CCH Alumni Association's Distinguished Alumni Award is one of the most prestigious awards that a BYUH graduate can receive. It recognizes outstanding former students of this institution. You are invited to submit nominations for outstanding alumni of BYUH-CCH to be recognized during the Jubilee Week this October 2005. Please submit your nominations on the attached BYU-HAWAII ALUMNI AWARDS NOMINATION FORM below with supporting materials by February 25, 2005 to the Alumni Office. Please include a summary of qualifications. Be concise, yet specific as to the nominee's accomplishments and how they meet the award eligibility. We apologize If you submitted a nomination via email or fax and ask that you please resubmit utilizing this form. Please clearly mark the award for which you are submitting your nomination: " Distinguished Alumni Award: awarded to BYUH graduates (from 1990 and back) who have given outstanding service to their professions, community, nation or church. For those from the BYUH target areas, preference will be given to those who "returned" to their home region after completion of their BYUH studies. " Service to Family Award: awarded to alumni of BYUH for outstanding service, rendered in a home environment, including extended family. It may be given to an individual or a family. " Honorary Alumni Award: awarded to non-graduates of BYUH who have rendered outstanding and significant service to BYUH. Please use the following considerations when submitting your nominations. The primary criteria shall be alumni who have given outstanding service to their professions, community, nation or church, and who have graduated from CCH-BYUH. Other criteria which will also considered during the selection process will include geographic representation (so that not all honorees are from the same geographic area; gender representation (spouses of any of the nominees who are BYUH graduates may also be considered providing you give supporting comments); major representation (so that not ALL are from one major); spirituality and service to church; returnability --- did this alumnus return home to their home country/region at some time after completion of his/her studies at BYUH. The following shall be considered "ineligible" to be considered: Previous University Award recipients, current BYUH employees, current alumni board members and missionaries. Awards are not given posthumously. While the following are not criteria for ineligibility, the below may affect the decision-making process: Members of the governing boards (including General Authorities) and those serving in an elective public office.
